Payment for routine office visits is requested at the time of service. Please let us know in advance if you are covered by an HMO (Health Maintenance Organization), PPO (Preferred Provider Organization), or other pre-paid health plan. Many of these programs require advance approval and/or referral from your primary care physician.
Since insurance coverage varies widely, we will make every effort to help you deal with your insurance carrier. Your insurance plan is essentially an agreement between you and your carrier, and you are ultimately responsible for all of your medical expenses and any balances not covered by insurance.
If you are eligible for Medicare, please sign an authorization form during your visit. The majority of your bill should be paid by Medicare directly to us. If you have supplemental insurance, please provide that information and we will submit the insurance claim on your behalf. You are responsible for your co-insurance portion of deductibles.
